Enrollment Procedures   Withdrawal/ Transfer Procedures

The following information is needed to enroll a student:

  1. Transfer/Withdrawal Form from previous school
  2. Transcript
  3. Standardized Test Scores – Must have HSA/MSA scores from previous MD Public School.
  4. Copy of IEP (if receiving services)
  5. Immunization Record - All students born in 1992 to present must have Hepatitis B Series and Varicella Vaccine or date of Chickenpox Disease.
  6. Birth Certificate
  7. Social Security Number
  8. Proof of Residency (all three required:  lease/deed-settlement papers(mandatory), BGE bill, driver’s license)
  9. Date Residency Verification (if applicable)  - must be notorized
  10. Physical Form (If it is student’s first time in Maryland school)
  11. Custody papers (if applicable)
  12. Current documentation of any chronic health problems

 

 

How to Withdraw or Transfer

  • Parent must contact the school IN ADVANCE to arrange for a Transfer/Withdrawal of Records.
  • A 48-hour notice is required so that the student can obtain their transfer grades and clear any obligations.
  • Withdrawal Form must be signed by parent/guardian.
