Enrollment Procedures   Withdrawal Procedures

The following information is needed to enroll a student:

  1. Transfer/Withdrawal Form from previous school
  2. Transcript
  3. Standardized Test Scores – Must have HSA/MSA scores from previous MD Public School.
  4. Copy of IEP (if receiving services)
  5. Immunization Record - All students born in 1992 to present must have Hepatitis B Series and Varicella Vaccine or date of Chickenpox Disease.
  6. Birth Certificate
  7. Social Security Number
  8. Proof of Residency (all three required:  lease/deed-settlement papers(mandatory), BGE bill, driver’s license)
  9. Date Residency Verification (if applicable)  - must be notorized
  10. Physical Form (If it is student’s first time in Maryland school)
  11. Custody papers (if applicable)
  12. Current documentation of any chronic health problems

 

 

To withdraw, the Parent/Guardian who enrolled the student will need to complete the form below by clicking on the link. The form can be completed and emailed, dropped off by the student or faxed to the Guidance Office. Please allow 2 to 3 days’ notice to receive your students withdraw packet.
